LIABILITY WAIVER - REQUIRED:
In consideration of my participation as a volunteer at the Twenty-Two Farms Animal Sanctuary, Inc., a Wisconsin non-stock corporation, located at 2022 Hwy 12, Baldwin, Wisconsin (“Animal Sanctuary”), and in consideration of the promises, covenants and agreements, the receipt and sufficiency of which are hereby acknowledged, the undersigned agrees as follows:

1. I am physically sound and have medical approval by my doctor to participate as a volunteer at the Animal Sanctuary. I do not suffer from any heart, lung or other medical condition or disease that might in any way hinder or prevent me from volunteering in any capacity at the Animal Sanctuary. I understand that I am personally responsible for my activities at the Animal Sanctuary and I am responsible for maintaining my own health.

2. I agree to abide by all the rules and policies of the Animal Sanctuary. I understand that a risk of injury is present when engaging in physical activity and I assume all responsibilities and risk of participation as a volunteer at the Animal Sanctuary.

3. I understand and agree that my participation is voluntary. I hereby release and discharge Twenty-Two Farms Animal Sanctuary, Inc., a Wisconsin non-stock corporation, Gina Alberti and Christopher Mizell (collectively the “Indemnities”) of and from any liability whatsoever hereafter arising from any loss, damage or injury caused as a result of or in connection with me
volunteering at the Animal Sanctuary.

4. I agree to indemnify and hold the Indemnities harmless from all losses, liabilities, damages, costs or expenses (including but not limited to reasonable attorneys' fees and other litigation costs and expenses) incurred as a result of any claims or suits that I, or anyone claiming by, under or through me, may bring against the Indemnities, to recover any losses, liabilities, costs, damages, or expenses which arise during or result from my participation as a volunteer at the Animal Sanctuary.

5. I also understand and agree that this release does not apply to any intentional, willful, or wanton acts on the part of any of the Indemnities. To the extent this release is deemed overbroad or unconscionable for any reason, it is my intent and understanding that this release will remain valid in all applications that are not deemed overbroad or unconscionable. I intend that statements made herein to be perpetual, and I will be bound by them unless and until expressly rescinded by me in writing.

TWENTY-TWO FARMS ANIMAL SANCTUARY, INC., GINA ALBERTI AND CHRISTOPHER MIZELL ACCEPT NO RESPONSIBILITY OR LIABILITY FOR ANY DAMAGE, INJURY, OR DEATH TO PERSONS OR PROPERTY.
